Our specialisations series continues with one of the most respected qualifications in German physiotherapy: the Bobath concept. Where Manuelle Therapie (Blog 48) is the backbone of orthopaedic work, Bobath is the backbone of neurological rehabilitation - the care of people recovering from stroke, brain injury and progressive nervous-system conditions. It is demanding, deeply rewarding work, and it opens a distinct and well-paid career path.
This blog explains what the Bobath concept is, how you qualify, why it is such a strong career and pay lever, and where it fits on the Jet Set Jobs pathway. Any figures below are indicative and reflect the 2026 picture.
The Bobath concept is a specialised, individualised approach to treating people with disorders of the central nervous system - conditions that cause movement problems, paralysis or spasticity. Rather than relying on fixed exercises, a Bobath therapist analyses how each patient moves and designs everyday, goal-oriented activities that help the nervous system relearn lost functions. It draws on the brain's lifelong ability to adapt (neuroplasticity) and is widely regarded as the most established approach for rehabilitating patients after a stroke, or living with Parkinson's disease or multiple sclerosis. It is applied not just in therapy sessions but as a 24-hour concept woven into the patient's whole day.
The Bobath certificate is a post-qualification further training (Zertifikatsweiterbildung), delivered to the internationally recognised IBITA standards and approved by the statutory health insurers under §125 SGB V. The adult basic course runs to around 150 teaching hours, usually delivered in several blocks over a number of months, with assessments that include a project, written questions and supervised patient work. Importantly, there is an entry requirement: you generally need at least one year of full-time professional experience after qualifying, and you must be working with neurological patients during the course. Course fees, like other certificates, typically run into the low thousands of euros and are often supported by employers - especially rehab clinics keen to build their neuro teams.
As with Manuelle Therapie, the real power of Bobath lies in billing. On completion you can apply for the statutory-insurance billing position KG-ZNS (Bobath) - the code used for neurological physiotherapy of the central nervous system. That makes you able to deliver, and your employer able to bill for, specialised neuro treatments a non-certified colleague cannot. But Bobath does something more: it opens the door to rehabilitation clinics and hospital neurology departments, which are typically tariff-bound (TVöD) - so the specialisation often comes with the security and structured pay of a collective scale.
There is a further pay dimension. Physiotherapists working with especially demanding neurological caseloads - severe neuro patients, intensive-care or complex cases - can be placed in higher TVöD groups (indicatively up to around €5,000 gross per month at the top end for the most demanding roles), well above the standard entry group. In other words, Bobath does not just add a billing code; it can move you into a higher-paid tier of clinical work.

Bobath is the best known of a small family of neuro-rehabilitation certificates that share the same billing category. Vojta focuses on reflex-based movement therapy (used with children and adults), and PNF (Proprioceptive Neuromuscular Facilitation) uses stimulation of the body's movement sensors to improve coordination. All three open neurological and rehab roles and are valued in tariff-bound clinics. Bobath is usually the natural first choice for adult neuro work, but a therapist who wants to specialise deeply in neurology often adds more than one over time.

If neurological rehabilitation appeals to you, choosing an early role in or near a rehab setting sets you up perfectly - you build the required experience, then add the certificate, then step into higher-value, often tariff-bound neuro roles.
