Manual Lymphatic Drainage (MLD) for Physiotherapists in Germany: A 2026 Guide

Continuing our specialisations series, this blog covers a certificate that appears in a huge number of German physiotherapy job adverts: Manual Lymphatic Drainage, or MLD (Manuelle Lymphdrainage in German). If Manuelle Therapie is for joints and Bobath is for the nervous system, MLD is for the lymphatic system - reducing swelling and fluid build-up in the body. It is one of the most useful and accessible specialisations you can add early in your German career.

What is Manual Lymphatic Drainage?

MLD is a gentle, precise, hands-on technique that uses rhythmic movements to stimulate the flow of lymph fluid and reduce swelling (oedema) in the tissues. It is the central component of Complex Decongestive Therapy (KPE), usually combined with compression bandaging. Developed in the 1930s, it is used to treat lymphoedema - for example the arm swelling that can follow breast-cancer surgery and lymph-node removal - as well as post-operative and post-traumatic swelling, chronic venous problems and lipoedema. Because so many patients across oncology, surgery and vascular medicine need it, MLD is a constant feature of everyday physiotherapy demand.

How you qualify - fast, and with no experience barrier

This is where MLD stands out. The certified course is set at 170 teaching hours under the statutory-insurance rules (Anlage 7 to the §125 SGB V agreements), typically delivered either as a single four-week full-time block or in two parts (a basic course and a therapy course) completed within about six months. Crucially - unlike Bobath - there is no requirement for years of prior experience: you simply need to be a recognised physiotherapist. That makes MLD one of the quickest, most accessible certificates to add soon after you gain your German licence. Course fees are set by the training providers (not by us) and are typically around €1,500.

Why MLD is in such high demand

The value of MLD comes from a combination of billing and sheer need. On completion, and after obtaining your billing permit, MLD becomes a separate statutory-insurance position you can charge for - like MT and Bobath, it turns a skill into revenue for your practice or clinic. But MLD has an unusually broad patient base: oncology aftercare, post-surgical recovery, vascular surgery and chronic swelling all rely on it. Practices with cancer-aftercare or vascular caseloads often cannot operate without an MLD-certified therapist, so holding the certificate makes you genuinely hard to replace - and strengthens both your job security and your pay negotiations. The 2026 statutory-insurance rate increase of about 2.5% raises the value of these treatments further.

MLD vs the other specialisations

Each specialisation suits a different stage and interest. MLD is the natural early choice: it is quick, has no experience barrier, and meets constant demand, so it is often the first certificate a newly licensed physiotherapist adds. Manuelle Therapie (Blog 48) is the orthopaedic cornerstone and takes longer. Bobath (Blog 49) is the neuro-rehab specialisation and needs about a year of experience first. Many physiotherapists build a portfolio over time - and a combination such as MLD alongside Manuelle Therapie is a common expectation for higher-responsibility roles in mid-sized practices.
