Unlike some countries where Indian healthcare workers face informal barriers, Germany operates a transparent, rules-based recognition system. If your Indian qualification meets the required standards — and a BPT from a recognised Indian university does, in most cases — the pathway to working legally and permanently is clear.
Germany's recognition authority for physiotherapists assesses Indian degrees against the German Physiotherapeut qualification standard. A Bachelor of Physiotherapy (BPT) from a university recognised by the relevant Indian regulatory council is generally considered comparable to the German standard, though some candidates may be required to complete an adaptation period before full recognition is granted.
The recognition process is called Berufsanerkennung — professional recognition. It does not happen before you travel. You arrive in Germany on a recognition visa and the formal recognition process happens on German soil during your adaptation period, while you are already employed and earning.
To work as a physiotherapist in Germany, you need to demonstrate German language proficiency at B2 level — both general German and professional language competence relevant to healthcare settings. German physiotherapists interact closely with patients, document treatment notes, communicate with physicians, and participate in ward rounds — all in German. Language is not a formality; it is a core professional competency.

The Section 16d visa — the Recognition Visa — is designed specifically for foreign healthcare professionals whose qualifications need to be formally recognised in Germany. It allows you to enter Germany, work in your profession during the recognition process, and complete the adaptation period. You do not need to obtain full recognition before leaving India. The recognition happens in Germany, while you are already employed and earning.

The programme is open to candidates below 43 years of age. If you are between 35 and 42, you are eligible — but time matters. The language training alone takes 10 to 12 months, and the full process from enrolment to departure is typically 18 to 24 months. If you are 38 or 40 today, the time to start is now.
